Take advantage of the Department of Pennsylvania's offer to pay for your MOPH Lifetime Membership Fee.  Just fill out, download and or print the MOPH Application Form below and attach a copy of proof of your receiving the Purple Heart, then send it in or take it to the Chapter of your choice for processing.  It's that easy..

Important Information concerning changes to MOPH/LAMOPH Membership.

The LAMOPH as the MOPH now have only Life Membership.  The Ladies can become Associate Chapter Members and hold any Appointed Position other than Adjutant and Executive Committeemen at the Chapter and Department Levels.  When the Ladies Annual Membership expires they can become Life Members by paying the Ladies Auxiliary an additional $30.00.  New Memberships among the Ladies Auxiliary cost $50.00, either as Ladies Life embers or Chapter/Department Associate Members.
